SPORTS MEDICINE INFORMATION

Athlete Information:

2026-2027 Pre-Participation Physical Form - All athletes in 7th-12th grade will need an annual physical on file prior to athletic participation. For PPE’s to be current for the 2026-2027 year, they will need to be completed AFTER May 1, 2026. Please upload your physical to RankOne AND turn it into the Athletic Trainer.


RankOne UIL Forms/Athlete Handbook - All athletes in 7th-12th grade and their parents/guardians will need to complete these online forms annually prior to athletic participation.

    • New Enrollments to Ponder ISD - contact Coach Hamm to get added to RankOne (must have been issued a new student ID#)


    Athletic Training Room Treatment and Rehabilitation Hours: (available for all athletes 7th-12th)

    • Before School: 7:30 am - 8:10 am in the HS Training Room

    • MegaLunch A Block: 6:45 am - 7:50 am in the HS Training Room

    • After School: 4:15 pm - 4:45 pm in the HS Training Room

    Athletes may also come in for treatment and rehab during the athletic period ONLY with Athletic Trainer and Coach's approval. Please reach out with any questions or if you need to be seen sooner.

    ATR hours vary during holidays and summer breaks. Please email the athletic training staff with any questions or need to be seen.

    Student Athletic Trainer Information:

    Student Athletic Trainers will have the opportunity to learn and practice new skills including, but not limited to: 

    • Hydration 

    • Basic wound care/first aid 

    • Assistance with emergency response 

    • Basic taping and bracing techniques 

    • Assistance with rehabilitation exercises and modalities 

    • Learning about anatomy, concussions and so much more!
